client agreement
I understand that a deposit of 50% of the total scheduled services is required at the time of booking. This deposit is non-refundable but will be applied to the total service charge. I agree to provide at least 48 hours’ notice for any cancellation or changes to my mobile appointment. Failure to do so will result in the forfeiture of 100% of the deposit. As a first-time client, I acknowledge that a non-refundable booking fee of $30 is required for the phone consultation. I understand that depending on the consultation time needed, this fee may or may not be applied to the service charges. I acknowledge that each client must fill out an individual form for their appointment request and consent.
